blepharoplasty

What is a blepharoplasty ?

Blepharoplasty is a surgery to remove the excess skin from the eyelids that are as result of aging thus achieving rejuvenation and remodeling of the periocular tissues. It is done primarily for aesthetic reasons, but sometimes the sloughing of the upper eyelids is so important that it can reduce the patient's visual field.
Changes in the upper eyelid are characterized by excess sagging of the skin which takes place over the years. If it's too exaggerated, the patient perceives eye fatigue and feeling of ocular "weight", sometimes expressing discomfort and headaches. If excess eyelid skin (what we call in ophthalmology "dermatochalasis") causes lowering of the eyelids ("ptosis") the patient would have a progressively limited visual field, whereby the natural shape of eyelashes is changed, which may cause them to enter into the eye and can lead to painful corneal ulcers and abscesses this being a medical cause for oculoplastic surgery.
In the lower eyelid the laxity of the structures causes two changes at palpebral level. On the one hand, the orbital fat hernia "drops out" of its usual structure, which gives rise to the known "bags" and "circles". Secondly, the sagging of the skin causes wrinkles, creases and descent of the lower eyelid.

blefaroplastia 01Blefaroplastia

Why do we do a blepharoplasty?

blefaroplastia 0

The eyelids are a critical part of facial expression and represent the focus of attention during social interaction. With the operation, we avoid changes in the direction of the eyelashes and strong eye discomfort thus protecting for our vision. The feeling of "heaviness, tearing and redness”, that the patient complains of is eliminated. On the other hand, and on the aesthetic level the "bags" that occur with age, give us an appearance of premature aging or tired eyes, which are inherited from parents to children. Plastic eyelid surgery has a very natural rejuvenating effect without causing drastic changes in your image and without visible scars.
blefaroplastia 03
Our aim is not "that the patient does not recognize himself", but to give you back the eyes you had previously, but without the heaviness and discomfort. The short duration (only one and a half hours) and a minimally invasive technique (performed under local anesthetic and the patient can go home afterwards) make this operation a very popular choice for our patients together with cataract surgery with multifocal intraocular lenses (Prelex Premium ). The surgery is to remove excess skin and adipose tissue. The scar in this intervention is invisible because it is hidden in the natural crease of the upper eyelid.

blefaroplastia 04We remove and restructure the fat bags from the lower eyelids, to give your eyes a more youthful and natural appearance.

The palpebral complex anatomy and its relation to the eyeball and associated structures, makes it essential that a specialist surgeon performs these interventions correctly and safely.

In the clinic Vista Felices we have a Plastic Surgery unit and innovative Oculofacial Aesthetic Medicine staffed by specialized professionals who will study the medical and surgical requirements and characteristics of each individual patient.
1) Our unit for Oculoplastic Surgery or Plastic orbital-facial surgery is responsible for the eyelids, orbits and tear ducts. These structures are affected by, inter alia, lacrimal closures (with constant flow of tears) anomalies of the eyelids (outward or inward edges, pain and redness), blepharitis (eyelid swelling, redness, itching), loss of elasticity of the skin around the eyes (bags, dark circles, Blefarochalasia), pterygium , eyelid tumors, warts...
Some foreign patients with fair skin are particularly vulnerable to potentially malignant lesions, especially in our country due to the number of sunny days and its intensity. These lesions must be investigated, and in most cases an incision is made and examined by a specialist in pathology. In our clinic, we have specialized in this field and we always try to leave a minimal scar. To achieve this, our  aesthetic and oculofacial medicine unit, combines surgery with minimally invasive techniques such as Botox, facial and periocular fillers with hyaluronic acid profile of the lips and nuance of wrinkles, chemical peel, tightening face threads , latest laser (Thermage, exilis ...) and techniques for tightening of the eyelid structures.
We develop personally appropriate protocols for beauty and health,or for special occasions, always using the latest technology and with the required documents for training in the relevant technique.Blefaroplastia 06

2) Our ophthalmology and eye surgery unit carries out monitoring in practice to guarantee good eye health . of patients who already have, or patients with a family history of glaucoma, cataract, age-related macular degeneration; short-sightedness, presbyopia or cataracts should have an annual examination of the retina, we also treat young people who want laser treatment, so they don´t have to wear glasses, etc. We approach all fields with the best medical technology to ensure good eye health (optical coherence tomography, Retinographie, fluorescein angiography, visual field measurement, tonometer by contact or air ...)

CHALAZION

What is a chalazion?chalazion

A chalazion is a lump that forms in the eyelids due to obstruction of the sebaceous gland ducts (Meibomian glands) that are found in the eyelid margin. These glands produce the oily component of the tear film, and when clogging accumulates it can leading to a painful inflammatory nodule that may affect the entire eyelid.

Why do some people suffer it frequently?

Certain people and ages have a predisposition to this pathology caused by hormonal changes that occur such as in adolescence or during pregnancy. There are also eye and skin diseases that act as risk factors, such as blepharitis, seborrheic dermatitis, acne rosacea ...

What is the treatment?

Approximately 25% of cases of chalazion disappear with conservative treatment. However, in many cases it becomes chronic and requires surgery. The predisposing factors must be studied individually in order to treat and prevent recurrences.

What does is the surgery involve?

Chalazion removal  is performed in our facilities under local anesthesia and the patient can go home after the surgery.

ECTROPION

 What is an ectropion?ectropion

Ectropion is the eversion (falling) of the free edge of the lower eyelid. This produces its exposure and loss of contact with the eyeball. It can affect both the upper and the lower eyelid.

Why does it occur?

The most common causes are: scarring (by burning or trauma), the senile (the most common due to aging of tissues of the eyelid, they get loose and unstructured) and paralysis (after a facial paralysis like Bell's phenomenon)

What symptoms cause ectropion?

The dryness of the exposed portion causes discomfort and redness of the conjunctiva of the eyelid, to varying degrees depending on the severity. The mucous membrane loses its structure and natural moisture where it forms a thicker and rigid layer. The dryness of the cornea and the constant irritation can lead to a predisposition to infections or corneal ulcers.

What is the treatment of ectropion?

Ectropión

On one side the hydration with artificial tears to prevent corneal injury, and as a final treatment reconstructive eyelid surgery. It is an outpatient surgery under local anesthesia (the patient can then go home) performed by an ophthalmologist specialized in oculoplastic.

SUBCONJUNCTIVAL HEMORRHAGE OR HYPOSPHAGMA

hiposfagmaSubconjunctival hemorrhage or Hyposphagma

Hyposphagma is a common cause of an ophthalmologist visit. It occurs usually after a vigorous rubbing of the eye, which causes that a small blood vessel breaks and bleeds, and it is visible through the conjunctiva.

Why does it often happen to me ?

There are patients who are susceptible, such as e.g. patients with high blood pressure or those taking anticoagulant drugs.

Hemorragia subconjuntival o hiposfagmaWhat should I do?

It is advisable to go to an ophthalmologist to assess the degree of bleeding, and especially if it is so strong that it can affect the cornea. After the eye exam, it is always advisable to measure several times the blood pressure at different times in order to exclude that the cause is a sudden rise in blood pressure, and if you are taking the above medications, it is sometimes appropriate to carried out blood test.

Diseases of the lacrimal ducts

vialagrimalThe constant tears can be due to an increase in tear production or, more often, due to poor drainage thereof.
Sometimes the tear duct is not permeable at one point of excretion, causing a blockage of the duct that carries tears from the surface of the eye into the nose, resulting in the constant falling of the tear on the cheek.
There are two types of occlusion of the lacrimal duct:
Acquired: the most common. Usually affects adults and can have many causes (infection, conjunctivitis, etc.)
Congenital: Affects children and occurs due to a membrane that prevents the passage of the tear to the nostril.

vialagrimalesquema

¿Cómo se manifiesta?

  • Blurred vision due to the tear film
  • Feeling moist eye
  • Constant tears rising in adverse weather conditions (wind, air conditioning, heating, etc.).
  • Repeated conjunctivitis (several times a year)
  • Lacrimal sac infections with fever, pain and inflammation

What are the causes?

The main causes are:
  • The Aging
  • Use of drugs, such as chronic eye drops, chemotherapy or radiotherapy on the area
  • Congenital, from birth, in children
  • Traumatism, burns ... in the area
  • Eyelid malposition

What is your treatment?

Depending on the location of the blockage, the ophthalmologist will decide a probe of the tear duct, a surgical procedure called dacryocystorhinostomy or install a Jones tube.

TUMORS

Any tissue of the eye can develop at any given time a tumor. These tumors can be benign or malignant, but all require a thorough and early investigation by your ophthalmologist.

EYELID TUMORS

tumores

Eyelid tumors are very common and occur as irregularities or "lumps", which can be confused with other injuries such as a stye or chalazion. They are usually benign, but require a biopsy for diagnosis.

TUMORS IN THE CONJUNCTIVA

These lesions usually appear in the whites of the eyes as a point or a small prominent area with blood vessels. Its assessment under the microscope is necessary and in many cases resection of the lesion to biopsy it.

SPECIFIC EYE TUMORS

These tumors occur on inside the eye. They must be diagnosed by a professional, because they often cause no symptoms. Sometimes they are malignant (melanoma Retinoblastoma ...) Sometimes they are also tumor metastasis of other tumors, benigns sometimes have treatment with a laser when they are vascular nature. Their early diagnosis is essential to ensure in many cases survival of the patient.

Why is the evaluation by an ophthalmologist so important?

tumores

Eye disease can occur at any age and often cause no symptoms until they appear and the changes occur. Blindness is preventable if diagnosed and treated early. Your ophthalmologist is the only specialist who can ensure appropriate investigation you need to deliver eye health and, both optical and medical or surgical provide their specific treatment.

It is recommended every 1 to 2 years to carry out regular studies, especially after age 40, although no eye symptoms appear.

Esthetic

WRINKLES AND "CROW'S FEET"

The "wrinkles" appear through a natural process of aging and drying of the skin. This process can be accelerated by many factors, among it emphasizes sunlight, weight loss and tobacco.arrugas

The wrinkles that are are formed around the eyes ("crow's feet") are the first to appear, with horizontal morphology and slightly curved upwards and downwards. If they are not very deep, you can treat them with middle and superficial peelings and Botox, with very good results. In those deep and marked can be used hyaluronic acid with low density. "Fillers" are also used to cover up the furrow, which is responsible in many cases for the dark circles under the eyes.

If dark circles and excess skin is already forming skin folds, these "wrinkles" cannot be eliminated with medical-aesthetic treatments, but with a surgical operation of short duration called "blepharoplasty".

EYE BAGS AND DARK CIRCLES

The puffiness and dark circles appear with age, giving us a sad and tired appearance..Today, there are treatments that "eliminate" these symptoms or "minimize" them, some medical (peelings, Carboxytherapy, hyaluronic acid ...) and other surgical (upper and lower eyelid surgery, Canthopexy ...) depending on the case.bolsasojeras

What are puffiness and why do they occur?

The "eye bags" appear over time through a process of aging of the muscles and the skin around the eyes, an accumulation of fluid by poor drainage or herniation of fat pockets that are placed under the eyes at an intermediate level. Depending on each individual case we can diagnose the cause and thus better tackle the most appropriate treatment. They usually have a hereditary component.

What medical treatments can be carried out to improve the dark circles ?bolsasojeras2

When the eye ring is caused by a markedness of the furrow in many cases, the filling with hyaluronic acid with a low density improves the appearance. Depigmentation peelings are also used to refine the typical bluish color.

In the case of bags, if they are also associated with an excess of skin, the upper and lower blepharoplasty is a very good option to remove the bags and give a healthy and natural appearance.

Which factors favor their appearance or worsen it ?

Little or poor sleep, excessive tobacco and alcohol, hormonal problems, some medications, damming tendency of liquids, allergies, pregnancy, kidney disease...

BOTOX (BOTULINUM TOXIN)

What is botulinum toxin?

It is a natural protein which is produced by a bacterium called Clostridium botulinum. Its use is now common for both major diseases such as the treatment of spasticity and in very small doses for aesthetic use. It reduces the action of the muscle to which it is applied.

How is it used in cosmetic medicine ?

BotoxTreatments in aesthetic medicine with this protein have increased greatly in recent years, although it is considered as a therapeutic pearl in the treatment and prevention of facial wrinkles today. When used on the face it gives a perception of youth and health, harmony and symmetry (commonly used in patients with facial paralysis).
In aesthetic medicine, we are always looking for the naturalness, safety and efficacy of the product.

What should I do if I want have this treatment?

First of all patients undergo, a medical examination, which also covers questions about medical history and any existing conditions and medications taken, also what the patients expectations of the treatment are. The needs of each patient are analyzed and all doubts and any riskc involved are always explained by direct doctor-patient contact. In many cases, the combination of aesthetic and medical procedures is the ideal to optimize results.

How long do the results last?

Botox

Usually 4-8 months, depending on the depth of the wrinkle and the patients usual facial gestures. For young people, more than once a year is not necessary, although not reapeating a treatment does not accelerate the progression of wrinkles. It is ideal as a preventive method before the wrinkles become too deep or too pronounced as the results over the years are spectacular. Remember, though, that what we do is relax certain muscles not paralyze them. This is the key to naturalness.

Can I combine it with other techniques?

Yes, and in many cases, they should be combined in order to obtain optimal results. The specialist in aesthetic medicine is the best one to advise you on your needs.

PEELING

What are the peelings ?

peeling

It is the application of a substance in the skin, in a controlled manner which will stimulate the regeneration and renewal of the superficial layers, wherein impurities are removed and cell regeneration is stimulated.

Why is it good to have from time to time a peeling ?

Because it keeps our skin smooth, moist, uniformed and toned, and prevents the appearance of wrinkles and facilitates treatment with other products that penetrate into deeper layers of the skin, so premature aging is prevented. We will feel the skin healthier and more natural after this treatment.

peeling1

At what age should you make a peeling ?

There are various types of peeling each with other effect. Because it is a preventive treatment, there is no specific age at which one should begin. A skin of a person with 25 years may be prematurely aged and on the other hand a person with age 35 has cared and moisturized. It depends on many factors such as sun, cold, tobacco, diet...

peeling2

What kinds of peelings are available?

They are diverse and each has a different origin. For example, there are those who comefrom sugarcane (glycol), milk (lactic acid) grape (tartaric acid) apple (malic acid) bitter almonds (almond acid)... Each has a different effect. You always have to adapte specially the treatment for each patient. de forma específica para cada paciente.

There are also certain combinations to effectuate luminosity, or anti-wrinkle, to treat rosacea or acne, antioxidants, with lifting effect ... It is best to consult the specialists.

How often can I repeat a peeling?

peeling3

It depends on the type of peeling, which we have used and the properties of the skin of the patient. There are those that should be repeated after six months and others that can be carried out every two weeks. It is important to plan in advance an event, like a wedding or communion to which the patient would go, in order to achieve the best result.

Is it possible to combine it with other techniques?

Of course. In fact, before other procedures such as fillers, mesotherapy, laser, IPL, Botox be applied, they are used to improve the texture and moisture of your skin.

Its effect is immediate and very natural.

Platelet Rich Plasma

plasma

Whatisplatelet-rich plasma and growthfactors?

Aging is a very complex process, where at the cutaneous level it is itsmost clearlymanifestation, so it is constantly sought ways to mitigatethis process. One of these alternatives is platelet-rich plasma facial infiltration, where you can start rejuvenation by using your own cells. It is a very simple procedure.

It is a simple outpatient technique based on the intradermal application of platelet-rich plasma (PRP) to activate by natural means the functions of fibroblasts, the cell responsible for determining the structure and quality of the skin Platelet-rich plasma is an autologous, non-toxic, non-allergenic preparation obtained by centrifuging one's own blood, the function of which is directly related to the release of growth factors of the platelets themselves. These growth factors are recovered and injected directly into the areas of the face, neck, décolleté, hands and the zones you need, with greater flaccidityand aging.

For what is plasma treatment rich in platelets and PRP growth factors used?

plasma2

The PRP not only has platelets, but also consists of: plasma, leukocytes, growth factors, protein secretion and all the components of the coagulation cascade. The PRP is one of the most modern techniques of cellular bio regeneration that uses the properties of platelets, a substance known as growth factor, which is a biological mediator that produces a range of effects at the cellular level (mother cells) where cell turnover and the regeneration is favored.

Growth factors are applied to the dermis of the skin to directly stimulate the fibroblasts to produce new collagen and elastin, promote the formation of new tissue, creating greater smoothness, luminosity and elasticity of the treated area. This is a fundamental advance especially in those people in whom cellular aging is accentuated and cell turnover slowed down. As one of themostimportantmanifestations of agingistheappearance of wrinkles, the use of growthfactorsalone, or in combinationwithother medicines, bymesotherapy, allowsus to provideagentsthatactivatetheregeneration of the skin and theproduction of collagen and elasticfiberswhich are necessary to replacethealteredstructure.

When is it recommended?plasma

This treatment is recommended from the age of 30, when the skin begins to lose its regenerative power, or simply when the signs of aging are visible. The improvement is noticeable already in the first few days and in its maximum after 20 or 30 days, this remains stable for about a year. The result of the last session with platelet-rich plasma must be repeated after the year, which is the time when a new, supportive kick-off with a new session is required.

Advantages of the treatment?

Intradermaladministration of platelet-rich plasma has beenshown to stimulatetheproduction of collagentype I bythefibroplasticcell. Intradermalinjection of growthfactors leads to remarkablechanges in aged skin, restores skin vitality, increases skin thickness, restoreselasticconsistency, improves vascular influx, stimulatessecretion, and improves skin smoothness and appearance ,Growthfactorsregulatetheremodeling of the epidermis and dermis and have a majorimpactontheappearance and texture of the skin.

FACE REJUVENATION

How does our skin age?

Rejuvenecimiento facial

Our skin consists of several layers, and each behaves differently with aging:
  • The Epidermis, or outer layer, over the years becomes, rough and not very bright. Fine wrinkles and blemishes gradually appear, which lead to the evenness and strength being lost.. The treatment would be to improve peeling, seek cell renewal and the use of depigmentation.
  • The Dermis, or inner layer is characterized by deep, marked wrinkles and laxity, which gives the face a lack of strength. At this level, a "compression" of the skin is necessary in order to achieve results.

    Rejuvenecimiento facial

What are the basic needs of the skin?

  1. Good facial care
  2. Moisture
  3. Antioxidants
  4. Sunscreen

Why is hygiene important for the face?

Facial skin care is important for achieving results in aging treatments. The surface layer of the skin is the first barrier to the entry of the products we use for aesthetic reasons and its penetration into the deep layers so that they can develop their effect; thus, the impurities and dead cells block the passage of the treatment to deeper layers, and therefore peeling and cleaning opens the way and allows penetration of the treatment. Peeling helps us to remove these impurities and gives us a way to act.

Why do we need hydration?Rejuvenecimiento facial 02

Dehydration is the first stage of skin aging... Lack of softness and elasticity, lack of luminosity and "dull skin", tightness, rough, flaky skin... favor the occurrence of "dehydration wrinkles" especially on the cheeks and around the eyes.

Why do we need antioxidants and sunscreen?Rejuvenecimiento facial 03

90% of the signs of premature aging are caused by free radicals.. The sun, tobacco, pollution, stress, poor diet ... promote the formation of these free radicals. Sun protection is thus the first barrier to prevent skin aging.

What treatments do we carry out in order to prevent skin aging?Rejuvenecimiento facial 04

First, we assess the patient and his or her individual needs. We take a medical history, in many cases a complete blood analysis to assess his or her health, and a study of their skin. Then we develop a concrete plan for their particular case.
We have peelings, masks and moisturizers, facial mesotherapy with multivitamins, mesotherapy with platelet-rich plasma, fillers and natural protein, threads for skin tightening, laser to remove stains and dirt... which ensure the improvement of wrinkles, tightening and moisture and make the skin look younger and more natural.
